ORDER. It is ORDERED that Petitioner must either: 1. Remit $5.00 as a filing fee to the Clerk of this Court, or 2. Complete and submit to the Clerk of this Court an "Affidavit in Support of Request to Proceed in Forma Pauperis." The Petition will remain CONDITIONALLY FILED.
